Our locations offer an exciting work life balance, with Dartmoor and glorious beaches on the doorstep and Plymouth, Britain’s Ocean City offering culture and excellent travel links.We are pleased to be able to advertise for a part time (24.75 hours) Band 6 Occupational Therapist to work in our wide ranging community teams based within Livewell South West.These teams include the Discharge to Assess team, Triage teams, and the Community teams which together form a rapidly evolving service to support, assess and rehabilitate people out of hospital into their own environment, (or that which is closest to it), and provide longer term rehabilitation and management for people in their own homes or care settings.Our team is a fully integrated health and social care MDT which supports timely hospital discharge and the evidence based ethos that ‘your own bed is the best bed’. The Community team provides therapy and management to support people to achieve and keep their optimum independence throughout their adult lives, taking referrals from individuals and a wide range of other services.We are looking for enthusiastic and flexible staff that are person-centre team players who will bring new ideas to support this proactive service.Livewell Southwest values its staff and makes investment in their development a priority.
