We promote a person-centred service so that our support is unique to each individual, based upon their own aspirations and needs.About youTrusted Expertsyou will be joining an experienced team of Dementia Advisers across the North Devon area, using a person-centred approach to provide expert local guidance, information and advice to people living with dementia and their family/carers.Better Together we pride ourselves on supporting people with an inclusive attitude and mindset is important to ensure we support everyone in the best way possible. There is an emphasis on partnership working and networking with Health and Social Care professionals,including the Older Peoples Mental Health Team, GP surgeries, Social Prescribers, Devon Memory Service and Social Services.
