B5 Nurse Advert Community Hospitals (Welland Ward, Johnson Community Hospital, Spalding):We are Lincolnshire Community Health Services NHS Trust (LCHS), the primary community healthcare provider in Lincolnshire delivering community-based services aimed at supporting people to manage their own health closer to home an reducing the need for people to go into acute hospitals. We are rated ‘Outstanding’ by the Care Quality Commission and are playing an integral part in shaping future health service provision countywide alongside partner health and social care organizations.The Welland Ward at Johnson Community Hospital, Spalding, is a 20 bedded rehabilitation ward which provides holistic person-centred care that enables individuals to increase independence, improve health outcomes and reduce unnecessary admissions to secondary care or long term care. This is achieved through timely interventions, rehabilitation, management of long term and limiting conditions and partnership working across the health and social care community.We have an opportunity for a number of Nurses to join our dedicated teams, with both full and part time opportunities available. This opportunity could also be suitable for a newly qualified nurse looking for the first step in their healthcare career.The successful candidates will be responsible for assessing care needs, and the planning, delivery and evaluation of nursing care using innovative research based effective practice. Good communication skills, both oral and written are also required, as well as being able to demonstrate evidence of team working.There will also be the benefit of a progressive development plan to enhance their current skills in a variety of medical competencies.
