Are you passionate about delivering a high quality patient centred careAre you motivated and dedicated to supporting patients with the transition from hospital to homeDo you want an opportunity to work in a service that is transforming how care is delivered in our communityWe are looking to recruit highly motivated Bank Rehabilitation Assistants to join our small and friendly team on Welland Ward.The therapy team provides care and rehabilitation for a variety of special areas including orthopaedics, palliative care, chronic disease management and other medical conditions. The service provides high quality care and works to an enabling philosophy to facilitate patients to achieve their maximal level of independence thereby facilitating discharge and reducing length of stay in hospital.Your role will include assisting patients to regain as much functional independence as possible through working on a patient rehabilitation goal approach. The post holder will be able to use skills to carry out simple assessments, follow care plans and prescribe routine equipment within the scope of the job and to a level of assessed competence.
