Do you wish to be a part of a team striving to make a difference in our community Are you looking for a new challenge Do you want to make a difference in patient careWe are looking for Band 5 Community Nurses to work within our Peterborough Neighbourhood Team’s.Cambridgeshire and Peterborough NHS Foundation Trust (CPFT) provides integrated community health services. We aim to support people of all ages to achieve the very best they can for their health and well being. To ensure that we achieve this goal we are looking for skilled people who share our passion for delivering an excellent service and standard of care.The permanent posts are eligible for a £1500 New Hire Bonus. An initial payment of £500 will be paid in the first 6 months and a final payment of £1000 will be paid 1 year after your start date. This will be pro-rata for part-time posts.In these roles, you would be working at part of the Neighbourhood team which includes District Nurses, Community Nurses (RN), Community Matron, Physiotherapists, Occupational Therapists, Mental Health Nurses and Team coordinators. You will link closely with colleagues including general practitioners and other members of the multidisciplinary team to support excellent nursing care at home, reducing the necessity for acute hospital admission.We welcome applications from those with current community nursing skills, newly qualified Nurses, Return to Practice Nurses and Registered Nurses wanting to develop these skills further and join our highly motivated and innovating physical health service.We are keen to support and develop our employees, allowing for career progression in encouraging promotional opportunities within the Trust. This includes the ability to develop and enhance clinical community skills such as wound management, palliative/end of life care, discharge support, diabetes management, catheter management and those living with long-term conditions at home. We offer a supportive preceptorship program for those newly qualified alongside an in-depth Clinical and trust induction for all employees.Full time and part time posts available.
