The team provide a community bed based rehabilitation service across two community care home settings in the Nottingham City locality, often patients are admitted to the community beds directly from hospital but some are admitted directly from their own homes after experiencing a crisis that does not require acute secondary care. The team work within the care homes and complete assessments, rehabilitation and discharge planning in a timely manner to support patients back into the community.The applicant will have a good knowledge of community services and an understanding of health conditions, especially those associated with ageing. You will work closely with Nurses, Occupational Therapists, Community Care Officers, Assistant Practitioners, Rehabilitation Support Workers and Physiotherapists as part of a Multidisciplinary team and have developed strong leadership skills in supporting a team.The role includes delivery of efficient and effective services in line with local, national and professional agendas.An understanding of Health and Social care provision and ability to work closely with partner organisations and other stakeholders is essential. You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation and Reablement programmes, as well as complex discharge planning experience.You will need to demonstrate high standards of evidence based clinical practice. You must have excellent interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.We are looking for enthusiastic, motivated, conscientious & experienced Mental Health Nurse to work with the established Urgent Care and Reablement Teams.
