We are looking for a motivated and dynamic practitioner to join our Primary Care Cardiac Service working with patients diagnosed with heart failure. There will also be opportunities to work across the wider service supporting people with cardiovascular disease who require rehabilitation and secondary prevention. The service aims are to increase access to services, reduce cardiac/stroke risk, manage symptoms effectively using evidence based practice and reduce unnecessary hospital admission. A comprehensive holistic assessment process is used to identify need and in collaboration with the patient a support plan negotiated..You will be a first level registered nurse with experience working in the field of cardiovascular disease. Previous experience working with heart failure patients is essential.You will have good communication and interpersonal skills and the ability to work both independently and as part of a team. You will be expected to work with a defined population and be able to successfully case manage your own patients and support band 6 colleagues to do the same. You will offer individualised care management to your caseload within their own homes and at other community venues. You will also link in with all relevant agencies and services to enable patients to manage their own health within your caseload area.You have a full, current driving licence and have access to a car.
