Health and Social Care OfficerNottinghamContract£17 P/HWe are currently looking for a Health and Social Care Officer for a role in Nottingham. The Health and Social Care Officer will provide advice, triage and co-ordinate information to enable decision making and risk management on behalf of citizens requesting/ requiring access to social care and health services, and to navigate citizens through to the right services based on an assessment of their needs.Principal duties and responsibilities:To coordinate telephone calls, enquiries and referrals from citizens, vulnerable adults, families and carers and to manage these in an effective, timely and customer focused way.To deliver an agreed level of customer service that reflects the vision, values, aims and objectives of Nottingham City Council and partnersTo manage specialist or escalated calls from Health and Social Care Support Co-ordinators, Health and Social Care Support Advisors or other colleagues.To have an understanding and knowledge of the variety of relevant services and their referral criteria available within social care and health to enable appropriate advice and guidance is given and to ensure this information is kept up to date.Liaising with and working with relevant clinicians, practitioners and health and social care provider services to coordinate appropriate care packages and ongoing care needs, this will include triaging and signposting citizens to other services, tracking andmonitoring pick up and handover of clients to the crisis response team.To make judgements regarding referrals and requests in relation to complex situations, safeguarding concerns and risk management by triaging and coordinating them to make sure that clients are seen by the most appropriate team / service at the right time tomeet their needs.For more information please call Sam on
