Youll work in a team of 6 people, all of whom are based in the Acute Mental Health wards across Norfolk. We support customers during extremely difficult and often traumatic times when they have been admitted to hospital. As our Support Coordinator, youll help change our customers lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. Youll create bespoke and personalised support plans with and for your customers.Each plan will look to meet our customers individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements.Youll support customers to have timely discharges and youll ensure their social needs are met, such as housing and benefits.
