We are looking to expand our team of Advanced Clinical Practitioners, by another 1 whole time equivalent post. This will enable us to continue outreach domicillary visits and admission avoidance work within our North Norfolk and South Norfolk localities via the Locality NEAT (Network Escalation Avoidance Team) models, in addition to cover for the 6 General Community Rehabilitation Units in Swaffham, Kelling, North Walsham, Norwich, Wymondham and Dereham. Each member of the team will have an allocated base, but will be required to cover flexibly across the service as and when required. The Advanced Clinical Practitioners will work closely with Supported Care in North and South Localities to enable vulnerable people to stay in their own homes.The team is highly supportive of regular CPD and learning, and will offer ongoing support to successful candidates. We have regular team meetings, teaching sessions, peer support and supervision.You must be comfortable with autonomous practice and be utilising skills of Advanced Clinical Assessment within their current or recent roles. An independent prescribing qualification or working towards is essential. If you are working towards qualification, this post would be Band 7 until able to practice independently. It is anticipated that for a new recruit, a period of time based within the units would be a positive opportunity for consolidation of skills, knowledge and confidence, prior to taking part in covering the outreach element of the role on a rotational basis. Whilst you will be required to work autonomously, there is always medical support available by phone in addition to regular ward based support.You must be resilient and share our enthusiasm for growth and development of the service. As such, you will understand that as an evolving service, change is both anticipated and embraced.
