Thats when it hits home.We have a fabulous job opportunity for you to join our awesome team as a Mental Health Lead Support Worker (internally known as a Support Coordinator). We support our brilliant customers with their mental health, learning disabilities, substance misuse, forensichistory and dual diagnosis. We have core properties within North Tyneside, which come fully furnished for customers leaving hospital, and we offer an outreach support service where we will support customers within their own properties/accommodation.We help our customers with a little bit of everything. This includes enabling out into the community, administering medication, supporting with their finances, meal preparation, helping with their benefits and much, much more!Youll help change our customers lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. Youll create bespoke and personalised support plans with and for your customers. Each plan willlook to meet our customers individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements.Youll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.We empower our customers to develop the independent living skills they need, to successfully move on.
