The starting salary for this role is £40,227 per annum for a 36 hour working week.The Transition Team, with office bases in Woking and Redhill, is a dynamic and vibrant countywide service for young people aged between 14 and 25. Although based within AdultSocial Care, our journey with the young person and their family starts much earlier at academic year 9.We work with young people who have a wide range of additional needs and complex conditions. We challenge inequality and traditional responses to servicedelivery and employ a strength-based approach to our work. Supporting them through the Preparation for Adulthood agenda, we ensure that young people have control of their life outcomes and aspirational goals that will support them into employment pathways,independent living, and meaningful occupation. We are remodelling approaches from traditional institutional approaches as we challenge inequalities that prevent youngpeople with disabilities and their carers from living a life free of discrimination and limitations.We understand the challenges of day to day occupational therapy and frontline social work and have put tools in place to improve your work/life balance andmake day to day tasks more manageable. We provide hybrid devices which have a touch screen and detachable keyboard which enable staff to access council systems when working directly with individuals. These devices allow colleagues to change the way they workand to be more agile.About the Occupational Therapy Role in the TeamWe have a small, stable, Occupational Therapy Service within the Transition Team and these opportunities do not come up often. We work closely with our OT colleagues in Childrens services,however our intervention and support are focussed on supporting young adults aged 18-25. This provides an exciting opportunity to support young adults at a significant developmental period of their lives.You will be visiting young people in their own homes,in tenancy based supported living and care homes and in educational establishments, including colleges and universities. You will carry out assessments and give advice regarding complex moving and handling arrangements, the provision of specialist equipmentand property design and adaptation. You will provide advice and support to both young adults, their family carers and independent support providers - often enabling them to move on to a more independent lifestyle.We provide support to Trainee and Student OTsas well as training to support you to provide high quality reflective professional supervision and Practice Education.About you.You will be a professionally qualified and registered OT with a proactive, person-centred, strengths-based approach to your work,a supportive team member and able to offer advice and guidance to OT and non-OT colleagues. You will have a minimum of three years contemporary post-registration experience, including routine experience of complex casework, risk assessment and safeguarding.Knowledge of social care policy and legislation is essential, as is a background in working directly with people with variable, unstable or challenging social care needs and clinical conditions. Experience of direct work with young people and young adultswould be beneficial. The ability to get the best out of others and work collaboratively with partner agencies is equally important, and you must have outstanding assessment skills.As a Senior Occupational Therapist, you will have proven skills and experiencein major housing adaptations and complex moving and handling assessments. You will have an eye for detail but also the ability to think out of the box to co-design creative solutions for the young person, which may sometimes be viewed as unconventional.Manyof the young adults we support have a degree of cognitive impairment and an applied knowledge of the Mental Capacity Act will, therefore, be essential.In return for your skills and commitment, we invest in our staff by offering a large variety of trainingcourses and staff benefits. Ability to complete person-centred and proportionate OT assessments including moving and handling risk assessments, recommending major adaptationand knowledge of specialist equipment to meet complex needs An in-depth knowledge of medical conditions and the likely impact on the individual as well as demonstrable knowledge of the impact of long-term disability on the individual, carer, and family. Knowledgeof support planning and carers assessments Continuing professional development.
