Thats when it hits home.Join our team as a Supported Living Coordinator based at Calder House in Mirfield, West Yorkshire! Our service is registered with the CQC and we have six self-contained flats on site for our customers.Our property is detached, set in its own grounds and has a large sunken garden and outdoor space. The garden is the focus of some of exciting plans for the months ahead!Our awesome customers have varied and diverse support needs and are referred to us by external support agencies. Amongst the kind of customers, we support are those with acquired brain injuries, moderate learning disability, low IQ or those on the autisticspectrum. Our customers are learning to live independently in the community and have a 1:1 support package to manage their aspirations, wellbeing, support needs, risks and safety. We support our customers to live their day to day lives as independently as possible, helpingwith everyday activities including daily living tasks, health, emotional and behavioural support.About youAs our Supported Living Coordinator, youre passionate and motivated. To you providing care and support is more than just a job, you make every customer feel valued and respected. Youre compassionate and use your unique empathy and creativity skills to help our customers reach their full potential. Treating your customers with integrity, you ensure that they maintain their dignity at all times.Youll help change our customers lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. Youll create bespoke and personalised support plans with and for your customers. Each plan willlook to meet our customers individual potential and long-term goals, making sure they meet safeguarding and confidentiality requirements. Youll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.To make sure that we keep our records straight, and customers safe, its really important that you are technologically savvy (or at the very least not a technophobe!) To make sure our customers are well supported, its important that youve had a job in asupport or care environment before.
