Brook Street Social Care alongside a nation-wide charityspecialising in mental health recovery who are currently recruiting for a Community Link Advisor. As a Community Link Advisor, you will be supporting people in their preparation for transition from living in secure settings in the Middlesbrough area, supporting them in achieving their goal of living a more fulfilling life whilst being integrated into thecommunity. You will work alongside colleagues in a multidisciplinary team to support the transition of people detained in Secure Settings into the community, and to and support the development of NHS Care plans and Risk Assessments and documentation. You willmanage a caseload for people who use the clients services and give all necessary assistance to those individuals to enable them to participate in community life. You will support and signposting to enable service users to access relevant agencies and supportservices e.g., counselling, housing etc.You will have proven experience of working with vulnerable adults struggling with their mental health, addiction, or substance misuse. You must have at least 6 months experience within a mental health service or offenders service. You must be non-judgemental,have excellent communication skills and organised.Benefits:- 25 days holiday per annum, plus statutory Bank Holidays, or time off in lieu if worked (pro rata for part time).
