We have a comprehensive understanding of this challenging but highly rewarding sector, this enable us to place the most suitable candidates with clients and find the best opportunities for candidates.We are advertising this fantastic opportunity onbehalf of a local authority in Liverpool who are looking to recruit a Qualified Social Worker to join a well experienced team within Adult Services.The Role Consists Of: Working closely with primary care/community services/hospital teams/voluntary communityservices. Liaising with other professionals and carry out joint needs assessments as appropriate. Ensuring assessments and care and support plans are person centred, asset based, and outcome focussed, concentrating on what the person with care and supportneeds, or carer can do. Meeting statutory requirements and follow best practice in relation to the Mental Capacity Act 2005. Ensuring that the outcome of all work with adults in need of care and support, and with their Carers, is to promote their wellbeing.Carrying out and reviewing needs Assessments & Care and Support Plans for individuals and their Carers in a timely and effective way ensuring that they are involved fully in the process.Candidate Requirements: Degree in relevant professional or equivalentqualification e.g. Certificate of Qualification in Social Work or Diploma in Social Work. Previous experience of working in Adult Services Team. Able to undertake high quality assessments, reviews and support plans for individuals.
