Job Description

Service Care Solutions are recruiting for an Outreach Family Support Worker to join Liverpool City Councils childrens & young peoples service.The purpose of the role is to target support to children who are at risk of family breakdown, struggling with emotional and behavioural difficulties, experiencing Neglect, domestic abuse, parental mental health and/or substance/alcohol misuse issues.The main areas of responsibility will be to:

Act as case holders for complex cases that step down from childrens social care (level 3/2). This will involve completing early help assessments, arranging Team Around the Family meetings and supporting families to improve outcomes for their children bytaking on the role of the Lead Professional and associated tasks.
Offer support on cases that become more complex and reach level 4. Support is continued on these cases to allow a smooth transition between early help and social care.
Provide intensive and targeted support to vulnerable children and young people. This includes bespoke packages and programmes. This also includes supporting adolescents with complex emotional and mental health problems.

To be considered for the position you must have the following:

Significant experience working with children and families.
Enhanced DBS
NVQ level 3 or equivalent

Full time position, 35 hours per week. Rate of paying from £16.79 P/H.
