Job Description

  • The post holder will monitor and maintain the highest standards across the Trust. The criteria and standards laid out in the Health and Social Care Act must be achieved and reinforced.
  • The post holder will develop effective working relationships internally and externally.
  • In particular with healthcare regulators (e.g., Care Quality Commission, Health Protection Agencies, regional and national networks, other Ambulance Services and other local healthcare organisations such as Strategic Health Authorities, Primary Care Trusts,Acute Trusts and will work closely with the Trust Medical Director, Director of Nursing, other executive Directors, General Managers and healthcare professionals within the Trust.
  • The postholder will work with national Patient Safety groups and experts and ensure continuity and that Trust Patient Safety practices and procedures are aligned to current national guidelines.
  • The post holder will be responsible for taking a project management approach to the introduction of the Patient Safety Incident Response Framework (PSIRF) and acting as an exemplar for the Team and the Trust in this form of themed investigation and learning.
  • The post holder is an ambassador providing advance theory and practical knowledge in developing a safety culture through sound management of Patient Safety standards and assurance using improvement methodology to learn lessons, identification of trendsand themes and achieve sustainable change.
  • Working proactively in partnership with the executive leadership team, operational top teams, clinical leads and selected external stakeholders to ensure Patient Safety arrangements are in place across the Trust; demonstrating political astuteness and anability to manage priorities with competing deadlines.
  • To lead the development, co-ordination, and implementation of effective risk management strategies within the Patient Safety Team and ensure the integrity of the Risk Register, working with colleagues to ensure an integrated approach to the safety culture.
  • The post holder will be an ambassador for the Trust, be credible and knowledgeable with the confidence and interpersonal skills to work with clinical, managerial and support staff at all levels across the organisation.
  • The post holder will have expertise in developing a safety culture through sound use incident and SI learning, knowledge of improvement methodology (learning lessons and achieving change) and have a proactive can do approach, be highly motivated and confidentin managing challenging and changing workloads.
Duties
  • Provide leadership, expert advice and resource on Patient Safety improvement methodology and delivery in relation to the project management of the introduction of PSIRF to the organization.
  • To manage the systems processes that support Patient Safety management ensuring compliance with external and legal requirements.
  • To manage the commissioning, management of internal and external investigations providing support and advice to facilitate their timely completion and maximise the opportunities to learn.
  • To work with the services to ensure that action plans derived from the management of the incident investigation are developed with a high level of efficacy.
  • Ensure that Patient Safety is fully integrated with all relevant Trust programmes and initiatives.
  • Champion and promote quality as a key focus within the Trust and across the health system to improve clinical productivity and effectiveness.
  • Provide senior project management, planning and change management knowledge and expertise to ensure that the wider quality improvement programme is successfully delivered in line with the programmes timescales
  • Contribute to the planning, managing, and overseeing the delivery of several multidisciplinary quality improvement projects and initiatives to achieve a beneficial change to Patient Safety in line with the quality objectives of the organisation and nationalsafety priorities.
  • Liaise with operational / corporate leads to agree and achieve deliverables, as appropriate.
  • Provide expert advice to senior clinicians/managers on management of Safeguarding including Root Cause Analysis, Incident investigation and quality improvements.
  • Ensure common understanding and shared commitment of team members.
  • Ensure that all Patient Safety risks are appropriately recorded and monitored at the appropriate level.
  • Pro-actively manage stakeholders, respond to and resolve conflict between different stakeholders when this arises through facilitation or other appropriate mechanisms.
  • Ensure the flexibility of the workstreams if required to meet conflicting/changing requirements.
  • Be responsible for own personal development to ensure the achievement of own objectives and that of the team.
  • Be able to explain strategy and organisational decisions in relation to Patient Safety matters to everyone in the organisation.
  • Represent the Trust at networks and conferences and other meetings regarding Trust developments and play an active role in sharing progress and learning with others
  • Maintain robust risk management and assurance processes that meet local and national standards and that are aligned to Trust governance processes and standards.
