Job Description

Seven Social Care are currently looking for a part time Social Care Assessor to join our clients team in Manchester. This is an interim post initially for 3 - 6 months with potential for extension.The role: You will be responsible for undertaking holistic Care Act 2014 assessments both within the community and hospital/intermediate care facilities. You will undertake home visits and carry out holistic strengths-based observations and assessments of citizensto support a determination of social care needsResponsibilities: Arranging timely support for reablement, packages of care, respite, carer assessments, equipment, and minor adaptations to support independent living, assistive technology and support carers and family members.Advising citizens and referring them to appropriate services based on the observation and assessment completed, ensuring that the provision of services supports citizens maximise their independence.Developing and implementing citizen support plans to meet identified social needs of individuals, enabling the citizen to achieve maximum health and well-being.Applicant Requirements: Qualifications: Hold or willing to undertake and achieve NVQ Level 4 in Health and Social Care within six months of commencing in post.Experience: 2 years experience in a similar role.Compliance: Current DBS check, right to work in the UK.Hours : 14 hours a weekWorking with Seven Social Care: We have been helping social care professionals find exciting new roles for almost a decade, so we know exactly where to look when it comes to sourcing the best opportunities.
