Youll be on the front lines, delivering a person-centered approach, empowering service users to set SMART goals, and providing valuable advice and emergency support.Youll manage referral appointments with the utmost dignity, ensuring service users receive the support they deserve. Clear communication is crucial as you collaborate with our support teams and external agencies.Your responsibilities also include coordinating referrals and establishing pathways for service users to integrate into the programme, offering them opportunities for employment, education, and training to ensure a brighter future.But your role goes beyond this; youll manage a caseload, conducting one-on-one sessions per week. Youll also provide drop-in and group activities within the structured program, focusing on making a positive impact in the lives of service users.
