Job Description

The Nightingale Hospital North West was part of the huge NHS mobilisation plan to deal with the growing number of Covid-19 cases and is a key facility in the regions response. It is located in the heart of Manchester, at the Manchester Central conference centre, formerly known as G-MEX (Windmill Street, Manchester M2 3GX). This facility is now being utilised to support patients who require rehabilitation after an acute admission prior to discharge.If is Nightingale Hospital North West sounds like an opportunity you would like to get involved with, we have two Patient Pathway bed flow manager roles available who will work in conjunction with each other over the two hospital sites.The Patient Pathway bed flow manager is a senior clinician (Band 6) responsible for the operational management of patient flow reporting to the Discharge Lead Nurse. Ensuring any relevant operational and escalation policies are implemented effectively feeding back any issues that may lead to amendment/improvement to our operational processes and systems.They are looking for someone who has the following

  • Clinical Experience as a NURSE or AHP preferred
  • Experience as a Band 6 or above and this must be for 12 months or more
  • Management experience within previous roles
  • You will need to
  • Be able to work 9am-5pm with a rota to cover 7 days a week
  • Ideally have experience with Patient pathway and Bed flow process
  • Work well with your own initiative as well as working within a team environment
  • Be comfortable making decisions independently
  • Main Responsibilities to include but not limited to:
  • This post holder will ensure effective and efficient flow, supporting the safe transfer of patients from an acute bed to ongoing assessment facility within the community, incorporating internal patient transfers according to direct need, repatriation and tertiary pathways based on accepted referrals both internal and external to the trust.
  • The post holder is responsible for supporting patients, families and carers to locate appropriate 24 hour ongoing care following an acute admission.
  • The post holder will be part of a multi-disciplinary team, and will also work autonomously, escalating unresolved issues to the Lead Nurse for IDT or the Operational Manager.
  • The post holder will ensure national quality standards are met within the remit and responsibility of the role and influence the site management provision.
  • The post holder will embed within working practice of the team a professional and productive interface between critical care and wards/departments internally and externally to the Trust to support the provision of a seamless service
  • Work proactively and in partnership with Trafford bed managers and the Site Management team to ensure timely transfers from the acute setting.
  • Identify and escalate delays in transfer of patients from the Wythenshawe Site to Patient Flow/ Operational managers as appropriate.
  • The post holder will embed within working practice of the team a professional and productive interface between MFT Sites and wards/departments internally and externally to the Trust to support the provision of a seamless service
  • Prepare regular situation reports throughout the day, regarding potential admissions and discharges, available beds and cancellations, for distribution to the senior management team
  • Ensure reference to electronic systems related to patient flow is made to ensure that the right bed under the right team and at the right time is in place.
  • Prepare and organise accurate information to be submitted for Operational meetings which are held at varying times throughout operational shifts. Attend Operational meetings to provide written and verbal updates and agree actions.
  • The post holder will be accountable to the IDT Lead Nurse / Operational Manager for the effective management of patient flow through the system optimising capacity for the efficient utilisation
  • Provide information to the IDT Operational Patient Flow lead as requested in relation to issues arising which are impacting on patient flow.
  • The post holder will actively assist in the management of activity and patient flow, working in partnership across site.
  • They will contribute to the development of operational objectives within Patient Flow
  • Work constructively with all members of the Patient Flow and Discharge Teams across site to ensure as a team we cover peaks and troughs in workload and maximise the potential of all our external and internal resources.
  • Be aware of budgetary implications associated with the service and ensure effective use of all resources to prevent waste
  • Be actively involved in identifying and participating in service improvements, initiating and communicating future developments to enhance quality of service provision and to enhance patient experience related to patient flow
  • Contribute to aspects of change management to support the effective delivery of patient services/patient pathways
