The Support 4 Families Team are essential in helping families get the right help at the right time. Thats why we are developing our early intervention and prevention services to makesure families can access a range of services and tiered support to prevent the need for statutory social work intervention.What We Are Looking For From YouYou will carry out assessments, care planning, person centre planning and with a focus on your care managementskills working actively with all partners to identify and meet individuals care support needs. You will be delivering direct interventions with families and using initiative and flexibility to be creative with work. Our supportive teams will actively encourageyou to develop your professional skills with access to regular support, supervision and training.You will need to be a registered Social Worker, have excellent skills in assessment and the ability to write concise records and reports.
