Qualified Social Worker - Southwark - Adults Hospital TeamTo provide a comprehensive, effective, integrated, systemically-informed social work service, within Southwarks practice framework, managing complex cases. To deliver excellent social work practice, assessing complex need, planning and working purposefullywith families, support networks and other professionals to manage risk, safeguard children and promote positive outcomes. To provide a social work service which is both relationship based and which responds in a timely manner within statutory guidelines. Towork collaboratively with the team, supporting the development of best practice and confident practitioners through coaching and mentoring of students, newly qualified social workers and less experienced practitioners. This post is located at Jenni Lee House,walking distance from the Kings College hospital site at Denmark Hill (London, SE5 9RS). This role is predominantly place-based in the hospital discharge team and visiting patients on the ward for complex discharge assessments and dedicated discharge planning,including mental capacity assessments and discharge planning meetings with medical colleagues, therapists and/or social care professionals and families. **NOTE: As mental capacity assessments and complex discharge assessments are carried out in person andwith discharge planning through winter pressures demanding a flexible approach to visiting patients on the ward, this role cannot be fulfilled remotely! This post requires 3-4 days per week office presence near the hospital, generally 1 day/week working fromhome for write-ups is unproblematic for this post.Benefits in this role:* Supportive team and management* Flexible working available* Long term contract* Progression opportunitiesYour main responsibilities include:* Hold and effectively manage a complex and varied caseload with appropriate supervision, guidance, and support, reflecting the function of the team and the Vulnerable People Service in accordance with policy, procedures, guidance and legislation.* Undertake work within legislative frameworks including the preparation of written statements and representations to the judiciary at Court hearings.* You will be accountable for complex decision making in relation to caseloads ensuring professional judgement, service user involvement, needs led assessment, critical reflection, and analysis to inform this.* You will identify and assess levels of risk and need (within statutory frameworks) in often complex situations. Undertake safeguarding investigations and deliver protective and/or supportive services for individuals at risk and maintain up to date assessments,care plans and reviews.* You will be preparing statements of need/care and support plans, in consultation with other agencies as appropriate, that reflect the wishes of the individual and lead to the commissioning of personalised packages of care.* You will undertake direct work with adults and their families in line with care plans.* You will work co-operatively with both internal and external colleagues across multi - agency boundaries.Essential requirements:* Social Work England registration* Eligible to work in the UK* 2 years+ post-qualified experience* Degree level or equivalent in Social WorkBenefits of working with Remedy
