Social Worker - Complex Care Team

We currently have an exciting post within the Complex Care Team, formally known as Continuing Health Care Team for a qualified Social Worker. You will have the opportunity to work in a small yet specialised team within adult services, alongside knowledgeableand supportive staff. The team currently consists of a Senior Social Worker, a Social Worker and an Assessor Care Co-ordinator.The primary role of the team is to assess individuals in line with SSWBA, having consideration for S.47 where an individuals needsmay exceed what the Local Authority is able to provide in terms of services to meet their needs. Where this is the case, you will be expected to work in accordance with the Continuing Health Care Guidance for Wales to ensure that the individuals care managementis legal and governed by the responsible body.Ideally the candidate will have some knowledge and/or experience of working with individuals with a Learning Disability, and be focused on a person-centred outcomes. The successful candidate will need to be experiencedin adult care and be competent in completing integrated assessments and care and support plans, as well as an experience and competence in carrying out capacity assessments, best interests and working generally within the Mental Capacity Act.The role involvescase managing some of the more complex cases within adults, learning disabilities and transitions, and you would be expected work to a high standard. In return, you will receive regular supervision, peer support, gain knowledge in a specialised area, and bepart of a successful project.You must possess a relevant qualification in social work and be a registered social worker.This post is a full time post based within the Continuing Health Care Team, initially until 31st March 2022.
