Pertemps Medical Professionals are sourcingSocial Workers with NHS Experience for our clients based in and around the Essex Region.ROLE:Band 6 Social WorkerTEAM:Community Treatment Team WORKING HOURS: Monday to Friday 09:00 – 17:00. Condensed Hours Available & Weekend Work Available. ASAP Start for a minimum of three both with the view of an extension. RESPONSIBILITIES: To be based in an office environment. To work as part of an MDT in an admission avoidance team. Assessing patients over the phone or assisting other members of staff with their assessments. Putting in packages of care, givingspecialist advice or arranging 24-Hour Care.SKILLS/EXP REQUIRED:To assess in a holistic way, triage calls and take leadership of the caseload from an AHP perspective, complete assessments and identifying the need.
