Job Description

About the team:The Support Plus team is a new service running within the Lewisham Forensic Community Team.Part of a pioneering programme funded by NHS England, the team will support forensic service users in the community instead of in secure hospitals. This can improve quality of life and reduce inequality, and allows hospital care costs to be reinvested into community services. Other teams in the programme have spoken of having a “genuine impact on service users lives” and the ability to “instil hope” and “witness real life changes”.About the role:We are looking for a full time, Band 6 Community Psychiatric Nurse to set up and be part of a ground-breaking new team, improving the lives of forensic mental health service users.The aim of this new service is to provide an intensively supported placement for men with a longer than average length of stay to gain and develop the skills necessary to live in the community. Focus will be on vocational and occupational skills, with social inclusion through a combination of support, access to community facilities, work experience, training and employment support. There is an emphasis on service user involvement and peer worker support.This is a rare chance to be part of shaping a new service, providing opportunities to develop your skills and your career. You will play a crucial role in the multi – professional team for this new service. You will need an Community Psychiatric Nurse with experience in either community mental health or forensic settings.The Post Holder will work as a member of the Lewisham Forensic Community Team and will solely work with the Support plus Service Users. The Post holder will need to be flexible, as the post will involve working across the 4 boroughs, however with a very small caseload of maximum of 15.You will be required to develop and maintain links across a wide variety of statutory and non-statutory organisations such as Probation, MAPPA, Jigsaw, Hostels and other voluntary and non-voluntary services. You will also be involved in specialist mental health assessments and the application of evidence based psychosocial interventions.This is a permanent post and as the team is part of an NHS England pilot due to end in March 2021, after which the post holder would be guaranteed a post in the community within the South London Partnership. We would also consider applications requesting secondment.About the location:We are based in Lewisham and the Kirkdale Resource Centre, we do not see service users at the team base. We see our service users in their accommodation whether this is supported or independent. Given this role accommodates all 4 boroughs if patients need to be seen at a hospital site this can be arranged.Kirkdale Resource Centre is based at 200 Kirkdale, Sydenham, London SE26 4NLOur Trust headquarters is located at Denmark Hill less than 5 minutes from the train station (zone 2).
