Service Support Worker - Assisted Discharge - CasualLocation: Based in Bexhill but covering East SussexContract: Casual (x5 positions)Hours per week: As and when required, Monday to Friday 10am to 10pm and 10am to 6pm at weekendsSalary: £17,508 per annum pro rata (£9.62 per hour)Shift patterns: 10 hrs per day Working days are variable Monday to Sunday (7 days per week with some weekends) Weve got an opportunity...Do you love helping people in need Are you looking for a rewarding role that could kick start your career in the health and social care sector We are looking for passionate and enthusiastic people with great interpersonal skills to join our Independent Living team in Bexhill-on-Sea. The primary focus of the role is to support the provision of our Assisted Discharge Service for non-Covid patients who are being discharged from either Eastbourne District General Hospital or the Conquest Hospital in East Sussex. Driving patients home when discharged (using Red Cross vehicles) and making sure they are safely settled at home with everything they need. You will also support them by signposting and referring patients to other sources of support in the community. We want all service users to have a positive experience in the transition from hospital to home. You will provide reactive, focused and flexible support to adults either following a hospital stay to enable early discharge when clinical needs have been met or to those living in the community to prevent hospital admission. You will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users needs in order to re-enable service users to remain in their own home and prevent hospital re-admission. You will respond to referrals from Clinicians and Community Health Professionals and will be required to have the ability to assess service users needs, complete and follow a support plan for the individual. In addition, you will be providing practical and emotional support in the service users own home and signposting to other services where appropriate to enable the individual to live independently in their own home.
