The all age Transforming Care Team has an exciting opportunity for a Service Finder looking to expand their knowledge and experience and who is dedicated and enthusiastic in the ethos of the Transforming Care agenda.Transforming Care is a national programmeimplemented by NHS England in response to the abuse of people with learning disabilities at Winterbourne View hospital in 2011. The programme objectives are to transform care and support for children, young people and adults with a learning disability and/orautism who display behaviour that challenges, including those who come into contact with the criminal justice system and those with a mental health disorder. The programme focuses on building up community capacity and reducing inappropriate hospital admissions.The team aim is to enable people to live ordinary lives in the community by offering support to people who are at risk of admission, to support the safe discharge from hospital and to build community capacity as a sustainable and suitable alternate to hospital.Itis essential you understand Transforming Care national programme and be able to contribute towards the delivery on NHSE requirements locally.You will be a key team member responsible for complex and personalised service finding, creative and bespoke care panning.It is essential to have experience in this area, including commissioning on systems. You will work within the discharge pathway for people in hospital and be pro-actively working towards accommodation solutions to facilitate safe and appropriate communitysolutions. The role will also include preventative work, so to identify services which will support people to live in the community to avoid hospital admission or readmission.Using your initiative to find solutions to problems in partnership and be accountablefor driving forward accommodation options for people on their discharge from hospital. You will be working at the forefront of best practice to deliver personalised support to vulnerable adults with the most complex care needs. Seeing the potential in peopleand taking a proactive approach to risk and problem solving will be essential to the role.Your work will involve collaboration with a range of stakeholders through co-production, engagement, and participation. You will be able to demonstrate your excellentcommunication skills and work in a personalised way, alongside stakeholders including clinical teams, commissioners, the voluntary sector, parent carers and of course the adults themselves.
