We have an exciting opportunity to recruit a SEND Transition and Support Coordinatorto join our multi-disciplinary ALS team. Responsibilities include being the key point of contact for families and external agencies as well as actively develop and maintain links with work experience providers. Working collaboratively with all stakeholdersto ensure the smooth running and successful transition of young people in and out of the SEND curriculum provision. It is essential that you have knowledge and experience of supporting young people and families, working as part of a multi-disciplinary teamand experience of leading meetings. You will also have experience of planning transition activities and experience in dealing with complex situations and communicating with local authorities is also desirable. The successful applicant will hold or be willingto work towards Level 4 qualification in social care / education / SEND support. Excellent written and verbal communication skills is important.
