LWEmployment Type: Permanent / Full Time / Hybrid workingAre you passionate about supporting young people as they transition into adulthood and independence Do you have experience in youth services or working with care leavers, and a commitment to empowering young people to achieve their goals If so, we havean exciting opportunity for you to join our 16 plus Independence Team (IT) at Lambeth Council.As a Social Worker in our IT team, you will play a key role in providing practical and emotional support to care leavers aged 16 and above, helping them to develop the skills, confidence, and resilience they need to live independently and achieve their aspirations.Key Responsibilities:

  • Provide holistic support to care leavers aged 16 and above, addressing their practical, emotional, and social needs.
  • Work collaboratively with care leavers, families, carers, and other professionals to develop and implement personalized support plans.
  • Support care leavers to access education, employment, training, housing, health services, and other relevant opportunities.
  • Advocate for the rights and interests of care leavers, ensuring they are involved in decision-making processes that affect their lives.
  • Ensure compliance with relevant legislation, policies, and procedures related to services for care leavers.
Requirements:
  • Relevant social work qualification (Social Work England).
  • Experience in youth services, care leaver services, or working with young people in transition.
  • Strong communication and interpersonal skills, with the ability to build positive relationships with young people, families, carers, and professionals.
  • Knowledge of relevant legislation, guidance, and best practices in services for care leavers.
  • Commitment to promoting the rights and well-being of young people transitioning to independence.
