Job Description

Join Our clients Team as a Multiple Disadvantage Referral and Move On Coordinator in Westminster!We are recruiting for a dedicated and compassionate Multiple Disadvantage Referral and Move On Coordinator to join our clients team in the Westminster area. Our service provides crucial support to individuals aged 18 and above, helping them navigate varioussupport needs and acquire the skills necessary to maintain independent accommodation in the long term.Job Details:

  • Position: Multiple Disadvantage Referral and Move On Coordinator
  • Working Hours: 37.5 hours per week
  • Working Days: Monday to Friday
  • Shifts: Week-by-week rotation between 8:00 AM - 4:00 PM and 10:00 AM - 6:00 PM
  • Duration: Approximately 2-3 months (short-term cover)
Role and Responsibilities: In this vital role, you will be expected to:
  • Incorporate move on plans from the outset, taking into account multiple disadvantage factors that might pose internal and external barriers
  • Handle referrals promptly and comprehensively, conducting trauma-informed and culturally sensitive assessments to consider all aspects of their experience
  • Manage a caseload of key 8 clients residing in the step-down service, conducting key work sessions to develop realistic and holistic move on goals
  • Respond empathetically to the challenges presented by complex needs in a residential setting
  • Lead the development of an accredited move on program that fosters the practical and interpersonal skills required for long-term tenancy sustainment
  • Conduct skilled and sensitive assessments with individuals who may be uncertain about accepting support and have had negative experiences with services in the past
  • Cultivate and maintain effective relationships with external agencies, including care teams, GPs/pharmacies, and other service providers, to ensure successful move on referrals
  • Participate in housing management procedures and weekly room checks on a rotational basis, addressing identified issues as part of residents move on plans
  • Deliver ongoing support through 1:1 sessions, group activities, and community engagement, focusing on residents strengths and helping them build confidence, self-esteem, interpersonal skills, and life skills for tenancy sustainment
  • Collaborate with residents to optimize benefits, ensure project income through rent and service charge collections, and minimise arrears and void loss
  • Utilise specialised knowledge of complex experiences and behaviours to contribute to safety planning and raise concerns when necessary, particularly concerning children and vulnerable adults
