The HealthPathways officer will offer one-to-one support to help participants overcome their health barriers, working in partnership with other providers to tailor packages of support suited to individuals needs allowing them to better manage their conditions and movetowards, or into, employment. The ability to innovate and inspire participants to achieve positive outcomes and act as an advocate of the services presenting a professional image of Moving on Tyne & Wear at all times is essential.Appointment to this post issubject to an enhanced disclosure endorsed by the Disclosure and Barring Service.About youThey are seeking someone with experience of working with vulnerable people in job-seeking and/or other community support settings. You will have worked in partnershipto deliver project outputs and outcomes. You must have the experience and ability to innovate and inspire participants to achieve positive outcomes.
