Job Description

Job Title: Community Psychiatric NurseTrust Location: Highgate, Camden & Islington NHS Foundation TrustPay Rates: £19 - £29About the Trust: Camden and Islington NHS Foundation Trust (CANDI) are rated Good in their most recent CQC report, providing high quality, safe and innovative care to patients in the community, in their homes or in hospital. What you will be responsible for:
  • Providing robust assessment of patients (by telephone or face-to-face) who have been referred to the CMHT due to presenting with mental health difficulties.
  • Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
  • Signposting to appropriate care providers (including primary and tertiary care). Attending daily Red Board meetings.
  • Raising concerns to senior staff for support and management. CompletingRiodocumentation and planning care for patients.
  • Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
  • Liaison with colleagues - nursing, occupational therapy, medics, psychology and admin.
The role will involve, but not to be limited to you performing above.Youll have the following qualification/experience:
  • A minimum of 6 months paid experienceas a Community Psychiatric Nursein anUKhospital environment within the last 3years.
  • Statement of NMC Registration
  • Degree /Diploma in Nursing
