Job Description

Band 6/7 Community Psychiatric NurseWe are seeking a compassionate and dedicated Community Psychiatric Nurse to join our dynamic healthcare team. The role involves providing essential mental health support and care within the community setting. As a pivotal member of our team, youll workcollaboratively with patients, families, and other healthcare professionals to deliver comprehensive care and support for individuals dealing with various mental health conditions.Hours: Monday to FridayResponsibilities:

  1. Conduct thorough assessments of patients mental health needs, formulate care plans, and implement evidence-based interventions tailored to individual requirements.
  2. Administer medications as prescribed, monitor their effectiveness, and educate patients and their families about medication regimes and potential side effects.
  3. Provide counselling, therapeutic interventions, and psychosocial support to individuals and families, empowering them to manage their conditions effectively.
  4. Offer guidance and education regarding mental health issues, promoting healthy lifestyles and coping strategies to enhance patients well-being.
  5. Liaise with multidisciplinary teams, including psychiatrists, social workers, primary care providers, and community organizations, to ensure holistic care for patients.
  6. Respond promptly and effectively to mental health crises, employing appropriate de-escalation techniques and coordinating emergency care when necessary.
  7. Act as an advocate for patients, ensuring their needs are addressed and their rights are upheld within the healthcare system and the broader community.
  8. Maintain accurate and up-to-date patient records, documenting assessments, care plans, interventions, and progress notes in compliance with regulatory standards.
Requirements:
  • Current and valid NMC pin.
  • Prior experience in psychiatric nursing or mental health settings is highly advantageous.
  • Excellent interpersonal and communication abilities to engage effectively with patients, families, and interdisciplinary teams.
  • Demonstrate genuine empathy, compassion, and sensitivity towards individuals experiencing mental health challenges.
  • Strong critical thinking and problem-solving skills to assess complex situations and make informed decisions.
  • Ability to adapt to changing environments and work independently within the community setting.
