About the team:The memory service offers a single point of access for comprehensive assessment, diagnostic and treatment services for individuals over the age of 18 living in Lambeth and Southwark who are experiencing cognitive decline consistent with an underlying dementia.The service works closely with Kings College Hospital and Guys and St Thomas Hospitals . In addition the service works in partnership with the Alzheimers Society to support people to live well with Dementia.About the role:We are looking to recruit an enthusiastic, confident and experienced Community Practitioner to the Lambeth and Southwark memory service.The post will have a key role in providing time limited support and interventions to people with dementia, their carers and relatives in both clinic and home-based settings and the post holder will liaise with colleagues within primary care, community health and social care services, voluntary organisations and the wider mental health of older adult service as appropriate. The post holder will be responsible for carrying out initial assessments (including standardised cognitive tests), formulating diagnoses and care plans with the support of the MDT, communicating with clients and their carers, discussing treatment options, and signposting people to relevant support services. The post holder will be skilled in communicating clearly and sensitively, both verbally and in writing. The memory service staff are expected to provide evidence based interventions and achieve positive outcomes for older adults and their carers these will be evidenced by performance indicators including activity data, targets and quality measures.About the location:This multidisciplinary team is currently based at Blackfriars Road in SE1 and clients are seen both in outpatients clinics at the Maudsley hospital and at Blackfriars road and in their own homes, according to need.
