Athona is currently recruiting for an ongoing locum Band 6 Registered Mental Health Nurse in West London.The post holder will be a member of the Primary Care Mental Health Team (PCMHT), providing effective day to day mental health care and management to clients with mental health problems presenting in Primary Care.The PCMHT provides a pathway for assessment, referral management and case management on behalf of patients with more complex mental health problems presenting in Primary Care.You will also work as a member of the Care Connection Team (CCT) within the transformed model of case management - Integrated Care Model. The CCT aims to prevent the escalation of service users health and/or social care needs thereby reducing the risk of emergency hospital admissions. The PCMHT provides a pathway for assessment, referral management and case management on behalf of service users with more complex mental health problems presenting in Primary Care.The post holder will offer short-term recovery focused mental health interventions to support this service user group within primary care settings.The successful candidate will be experienced in Continuing care; knowledge of Continuing Healthcare (adult framework) is required. Will be remote working so needs to be competent/confident carrying out assessments/reviews etc over Teams.
