In collaboration with Dementia UK, Home Instead Hammersmith and Chiswick are looking to recruit an exceptional, registered nurse with a passion for, and experience of, providing relationship-centred dementia care to join our team as an Admiral Nurse.Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to families involved in the care of a person with dementia with complex needs.The successful applicant will be an NMC registered (adult/mental health/learning disability) nurse, with significant post-registration clinical experience of working with people with dementia and supporting their carers/supporters in different settings,along with formal knowledge of dementia acquired through clinical post-graduate training/education.Role SummaryThe role is flexible, with a hybrid model; you will be based at the support office in Chiswick, London with flexibility to work from home.The Admiral Nurse service will focus on building the knowledge, skills and confidence of professional carers and the wider family to support families caring for the person living with dementia to remain in their home environment for as long as possible.The Admiral Nurse will act as an internal specialist resource for Home Instead Hammersmith and Chiswick, supporting the best practice of those involved in the care of the family affected by dementia as well as developing assessment and care planning toolsfor use across the network.The Admiral Nurse will provideadditionalsupport to that already offered by Home Instead care staff and will:
