Job Description

Areas covered: Llanelli, Burry Port, Kidwelly, Pwll £10 per hour (block contract) 25p per mile travel allowanceDrivers onlyWe are looking for a Senior Support Keyworker to join the Fulfilled Lives Service Our skilled team support individuals living with different forms of Dementia, mental health and other cognitive impairments.We will nurture talents and skills that you already have that will make such a difference to someone who needs your supportDUTIES: As a Keyworker you will be

  • Looking after your own caseload of service users.
  • Planning your own rotas
  • Responsible for assessing their need for any TECH support and any cognitive and physical needs they may have, and you will be putting effective solutions into place.
  • You will receive full training to increase your skills and identify your strengths,
  • You will provide person-centred care for each individual service user, helping to maintain their independence and support them to live well in their own homes.
  • You will be liaising with Multi-Disciplinary Teams to organise the care needed for the service user.
  • Participate in training activities and meetings as required.
  • Responsible for maintaining the relevant systems of paper-based and electronic documentation.

REQUIREMENTS:
  • Experience of working with Dementia service users is essential
  • A Current Driving licence and own car is essential
  • A good working knowledge of Dementia / Alzheimers and its effects
