Job Description

Areas covered: Llanelli£10 per hour (block contract) plus 25p per mile travel allowance Drivers Only We are offering a full-time permanent role to the right person who wishes to work with our Reablement Service in the Llanelli Area. Our Team work with Service Users who need that extra bit of confidence building, providing that little bit of help to maintain their daily routines. Your role is to reskill them in old and introduce them to new activities that they thought they couldno longer take part in. You will be responsible in finding activities for them to attend, supporting them to become independent and giving them the skills to do so. We will nurture talents and skills that you already have that will make such a difference to someone who needs your support! About the Service:

  • Focuses on a persons strengths to promote and maximise independence and wellbeing.
  • Rebuilds confidence after a spell of illness, deterioration in health, an injury, a hospital admission or an acquired disability.
  • Different to traditional home care as it involves care providers standing back and encouraging the promotion of self-care skills that may have been lost.
  • Supports people to regain or retain skills to enable them to manage with minimal or no support.
  • A focus on restoring independent functioning, rather than resolving healthcare issues.
  • Aims to prevent re-admission to hospital or premature move to a care home.
  • Is provided in a service users home
  • About setting and working toward meaningful goals.
  • Aims to reconnect people with their communities to reduce social isolation.
  • Can help to reduce the amount of care the person needs from carers and family.
As a Reablement Support Worker you will be
  • Looking after your own caseload of service users.
  • You will be responsible for assessing their need for any TECH support and the cognitive and physical needs they may have,
  • and you will be putting effective solutions into place.
  • provide person-centred care for each individual service user, helping to maintain their independence and support them to live well in their own homes.
  • There is an element of personal care in this role, as some Service Users need that extra support.
  • You will be liaising with Multi-Disciplinary Teams to organise the care needed for the service user.

Your Requirements:
  • Experience in working in a care role is desirable
  • Current Driving licence and own car is essential
