Job Description

Community Link Worker | EH54 based working Livingston, Bathgate and throughout West Lothian as required | 37.5 or 18.75 posts available | £22,425 (pro rata for part time)** Training provided**Contributory Pension** Travel Allowance ** Full and part-time hours are available**Our client is a charitable company who have been delivering community based mental health services since 1983 and worked in the West Lothian region since 2014.An opportunity has arisen to work in an innovative multidisciplinary service within community wellbeing hubs in Bathgate and Livingston. Presently some of your time may also be spent working from home.Our client is looking to recruit additional community link workers to deliver person centred individual support to people in contact with a G.P. practice team who may be experiencing complex social circumstances that are having a negative impact on theirhealth and wellbeing.You will support people to help them identify personal outcomes and priorities that they would like to address, and link them to local and national resources and services as required.Are you the right person for the job

  • Previous experience of working with people in demanding situations would be desirable
  • An understanding of mental health issues
  • Good communication skills
  • Builds rapport easily
  • Able to work independently in a proactive manner
  • Able to sustain contact with individuals to encourage and motivate them
  • Confident in working closely with clinical staff, G.P.s, practice staff and community agencies.
  • Able to maintain accurate records
  • Basic IT skills, working knowledge of office 365 desirable
  • A full driving license and access to a vehicle for work purposes
What will your role as a Community Link Worker look like
  • To develop an initial rapport and asses individuals using a standard format to determine their current and relevant past experiences to enable and empower them to identify their priorities and goals
  • Following initial discussion identify a range of options that could assist the person to improve their mental health and wellbeing
  • Following initial discussion liaise with other members of the clinical team to seek additional feedback where necessary
  • Prepare and facilitate follow up appointments to support people to take an active role in the management of their health and wellbeing
  • Following first meeting agree with people initial actions to be taken and follow these through in line with agreement throughout duration of support
  • Active delivery of individual and group-based psycho educational courses/interventions, including Living Life to the Full
  • To assist people to be more aware of paper based and online self-management tools
  • To assist people to be more aware of and encourage them to use community-based resources and pursue their interests and aspirations
  • To signpost and/or directly assist people to obtain advice and information on issues of concern/interest to them
  • To maintain positive working relationship with other team members, including nurses, psychologists, occupational therapists, admin staff and Wellbeing Practitioners
  • To work flexibly to meet the needs of peoples required support
  • To complete standard paperwork/online forms following initial contact and follow up appointments
  • At end of each shift ensure all relevant paperwork and online communications are completed, as well as ensuring confidential materials are securely stored and potentially assist with closing of building
  • To keep accurate and up-to-date written records of work in keeping with policies and procedures
  • To gather, record and collate data, including case studies/patient stories in a prescribed format for monitoring and evaluation purposes
