An excellent opportunity has arisen for a dynamic Respiratory Nurse Practitioner to work within our multi-disciplinary Community Respiratory Team. The service provides a 2-hour rapid assessment Hospital Admission Avoidance and Early Supported Discharge for patients with exacerbation COPD along with nurse led respiratory optimisation in patient home and clinic settings. The service also delivers a supported hospital discharge service for patients admitted due to Covid-19.Clinical leadership is supplemented by a respiratory Consultant working in the service. A high level of clinical decision-making is required for this post. Therefore sound theoretical and practical knowledge relating to this speciality is required from an expert practitioner who strives to maintain clinical excellence.The role will involve the assessment of patients in two local hospitals for the suitability of providing early supported discharge and the continued case management during their exacerbation in the community. You will need excellent communication skills and will be able to demonstrate your ability to make clinical decisions autonomously at pace. A willingness to undertake clinical audit and research and to gain an understanding of the local and national issues relating to respiratory disease in order to optimise the management of this group of patients is essential. Given the community-wide nature of this role, a high degree of mobility is required.
