Job Description

ACHD Coordinator JOB SUMMARY To be responsible for the delivery of the Adult Congenital Heart Disease (ACHD) MDT for the North West of England, Isle of Man and North Wales that meets the necessary standards set out by NHS England. The post holder will be responsible for monitoring allpatients that require an MDT decision and are tracked through their pathway to enable waiting time targets to be met. The post holder will liaise directly and communicate effectively with all clinical teams and departments across the ACHD North West CHD Networkto ensure the patient pathway is managed efficiently and to a high standard. The post holder will also provide administrative support to the ACHD nursing team.Location: LiverpoolJob Type: TemporaryDuration of booking: Expected to last 2-3 months with possible extensionProposed start date: ASAPPay Rates: Up to£14.00 per hour Umbrella or £12.75 per hour PAYE inclusive of holidaypayHours / Working Days: 37.5 hours per week / Monday to Friday, 9am - 5pmSector: HealthcareBased: Office / HospitalSkill knowledge and experience

  • Previous NHS experience at administrative/ secretarial level

  • Expertise in a range of electronic systems including Microsoft Office
MAIN DUTIES AND RESPONSIBILITIESThe post holder will be responsible for:
  • Managing the list of patients waiting to be discussed in the ACHD MDT providing full administrative support for the weekly ACHD MDT meeting.
  • Ensure MDT outcomes are documented appropriately and MDT outcome letters are generated and sent out in a timely manner to ACHD teams across the network, GPs and other clinicians/departments, as appropriate.
  • Responsible for managing all practical aspects of running the ACHD MDT meeting.
  • Be responsible for ensuring all necessary clinical data is available to ensure effective discussion can take place.
  • Be responsible for maintaining ACHD MDT records in line with NHSE CHD Standards and compliant and adhere with the Data Protection Act.
  • Ensure effective communication regarding all aspects of the MDT process.
  • Dealing with complex clinical information and using communication skills to ensure equity of service delivery to patients.
  • Be familiar with different Trust IT systems and work intelligently across all hospital sites.
  • Will familiarise themselves with different PACS systems and image storing facilities across the Network and will be fully functional in the use of the Image Exchange Portal (IEP) across the region.
  • Assist with and undertake audits relating to ACHD-MDT activity and report to the service line manager.
  • Ensure that all MDT data is collected accurately and is up to date. Data to be available for NHSE peer review and on request from NW CHD Network as required.
  • Monitor the patients progress on the pathway and check daily to ensure that escalation points are adhered to.
  • Highlight to Service Managers for escalation, those patients whose progress is likely to fall outside the agreed timescales.
  • Be responsible for inputting accurate and timely data into the ACHD-MDT database for the North West of England, Isle of Man and North wales. Support clinical teams in developing clinically useful, accurate and complete datasets and reports for the CHD Team,Trust managers and when required NHS England.
  • On the rare occasion that complex cases are discussed and the patient requires a rare procedure to be performed that necessitates a referral to another centre for a second opinion or treatment - the ACHD Coordinator will be responsible for ensure the smoothtransfer of data.
  • Develop and regularly review a detailed, standard operational policy (SOP).
  • Support the North West ACHD patient helpline, taking messages off the answerphone and record accurately.
  • Signpost any urgent messages to the ACHD nursing team in a timely manner.
  • Action patient attendance on PAS and send outcome letters to the GP.
  • Manage any general administrative queries from the helpline
  • Arrange ACHD nurse led clinic appointments
  • Coordinating and completion audio-typing of clinical and non-clinical correspondence within the service line
