Integrated Older Adult Service - Stoddart HouseRNMH or RNLD Matron PostThe future will be about the local care economy organisations working together to deliver the best quality experience and outcomes for patients in a sustainable way. We are at the forefront of shaping healthcare delivery locally. The Trust is an active member of the developing Integrated Care System across Cheshire and Mersey.Mersey Care NHS Foundation Trust and Liverpool University Hospitals NHS Foundation Trust are working in partnership to develop a brand new service for older adults to improve patient outcomes, reduce lengths of stay and bring together a range of expertise to deliver an integrated physical and mental health care model.The service is based on a Nursing and Therapy reablement model and our workforce will consist of clinical experts from Community Intermediate Care Bed Base, Community Therapies, Integrated Care Teams, Community Nursing, ICRAS, Psychological Therapies, Learning Disability and Mental Health expertise.Opening in March 2021, the first phase of the service will be based within the Aintree Hospital grounds and the estate is currently being designed and renovated to meet the clinical model and the needs of this patient group.This is an exciting time to become part of a fully integrated service aimed at recovery of the whole person. Our combined expertise, revised pathways and community connection will enable more timely discharge in a holistic way.Reporting to the Clinical Services Manger, the successful candidate will support the development and implementation phase and the further development of the model of care once fully established. The post holder will work with, and through, the Community Divisional leadership teams with a total commitment to our patients and service users.Candidates for this role should have experience in leading care delivery in the community or acute sector and have an ability to motivate and engage multi-disciplinary teams in improving services.
