Firstpoint Complex Care presents a fantastic opportunity for a Care Co-ordinator within our Complex and Continuing Healthcare team based in central Leeds.As a Complex Care Co-ordinator you will provide a essential link between service user, care team, commissioner and family. The role is based around the recruitment and rostering of a robust and flexible workforce to meet the needs of Complex Care packages across Yorkshire and the Humber. Applicants must be well organised and capable of working efficiently both as part of a team and using their own initiative.Account management and workforce planning experience is essential as is a background in either care delivery or healthcare recruitment. You will assist the Registered Manager to ensure that the service is safe, effective, caring, responsive and well-led, meeting a minimum standard of good with CQC. You will be a confident and experienced care or recruitment professional, eager to develop new skills and willing to push personal development boundaries outside of your comfort zone. Crucial to the role is the ability develop rapport and manage a team of care staff in a professional manner with a firm-but-fair approach.The ideal candidate will have the ability to build strong relationships with all stakeholders including care workers, clients, families and commissioning bodies. You will be quick-thinking and highly self-sufficient with keen attention to detail and ideally possess experience of person-centred care plans, risk assessments and other relevant documentations such as MAR charts and equipment logs. Personal drive and the ambition to deliver a top performance are a must, as is a genuine passion for delivering outstanding care.You will be working for one of the UK’s leading Care & Nursing providers. Quality and an exceptional customer experience is at the centre of everything we do.Key Tasks:- To work strategically with your Registered Manager to deliver growth targets.- To create effective and lasting relationships with all stakeholders, in particular service users, care workers and families.- To roster all care packages efficiently with minimal errors, ensuring both workers and care co’s and customers are clear at all times who is rostered to deliver package.- To manage client expectations around preferred workers whilst ensuring both short, medium and long term contingency cover is available for all packages at all times- To ensure all documentation relating to care packages are informative, detailed and up-to-date in line with the customer’s health and social needs.- To identify and recruit a cohort of experienced care and support staff, competent and capable of delivering an excellent service.- To ensure that all care staff are trained and competent in all aspects of mandatory training at all times as required by CQC.- To ensure, in liaison with the clinical educator that all care staff possess the required clinical training and are competent at all times to deliver the package of care.- To understand all regulations, compliance & risk assessments involved in new and potential package opportunities.- To assist with the completion of care needs assessments, risk assessments and supporting documents, helping to create engaging interactive care plans.- To manage your own time effectively to meet deadlines.Personal Specification- 2+ year’s experience delivering a service in the healthcare sector- Sound prior knowledge of the care industry- Good standard of general education- IT skills (MS Office - Word, Excel & some Powerpoint)- Excellent communication skills- Ability to effectively manage time and influence others to meet deadlines- Self-starter with high personal drive & energy- Exceptional customer service skills- Hungry for success- Clean DBS- Full UK driving licence.Benefits- Competitive salary from £18,000 to £24,000 per annum.- Working Hours or 8.30am-5.30pm Monday to Friday- Shared evening and weekend on call duties- Opportunity to gain accredited Trainer/Assessor Qualification- 28 Days annual leave including Bank Holidays.
