Are you looking for an exciting and challenging role working in nephrology and transplantation which will improve the pharmaceutical care provided to renal patients in both the in-patient and out-patient settingThe LTHT Renal service provides high quality care for the population of Leeds and the surrounding areas as one of the largest regional, specialised services for dialysis, renal transplantation and nephrology.Working closely with the multidisciplinary team, our renal pharmacists and technicians are integral to the safe and effective use of medicines, and lead on significant developments and innovative strategies to improve the medicines service available to our patients and other users.This new post has been created to help establish pharmacist-led medicines optimisation outpatient clinics to support patients to get the best from their medicines, including the dosing and supply of immunosuppression to transplant recipients.Activities will also include attendance on consultant ward rounds, provision of expertise into governance and strategic development of the renal services and the education and training of pharmacists and other healthcare professionals in the field of renal and transplant medicines.
