This is an exciting opportunity to join a dynamic and values driven Care Home Support Team in East Dunbartonshire. You will provide professional leadership and operational management of the Care Home Liaison Nursing (CHLN) service to deliver enhanced clinical care and provide quality assurance for the residents in East Dunbartonshire Care Homes. You will also provide clinical and professional advice and support to the Care Home Support Team which a new Multi-Disciplinary Team consisting of Nurses, Social Workers, Commisioning Officers, Review Officers, Pharmacy Technician and Dietician. The team is currently expanding and the postholder will be involved in developing strategies to support the provision of high quality care.
