Job Description

This is an exciting opportunity to join a dynamic and values driven Care Home Support Team in East Dunbartonshire. You will provide professional leadership and operational management of the Care Home Liaison Nursing (CHLN) service to deliver enhanced clinical care and provide quality assurance for the residents in East Dunbartonshire Care Homes. You will also provide clinical and professional advice and support to the Care Home Support Team which a new Multi-Disciplinary Team consisting of Nurses, Social Workers, Commisioning Officers, Review Officers, Pharmacy Technician and Dietician. The team is currently expanding and the postholder will be involved in developing strategies to support the provision of high quality care. There will be a requirement to hold a clinical caseload.Essential Qualifications and Training

  • 1st level registered nurse registered with NMC
  • Management Experience
  • Evidence of relevant training, education and experience at team leader/supervisory level within the NHS
  • Experience of Multidisciplinary work within organisation, across organisations and with external organisations and service user groups.
  • Experience of change management including supporting the design and delivery of new services
