Job Description

Deliver effective and high quality occupational therapy to patients in a home environment,or in the community, to help maximise their function and level of independent living.Duties of the Occupational Therapist

  • To conduct an accurate holistic assessment of patients needs in accordance with approved model of care.
  • Assess, discuss and agree upon functional goals related to daily living that are important to the patient such as washing, dressing, domestic chores, bathing, driving, preparing and eating meals, or participating in work, school or leisure activities.
  • Educate patients and give advice on the safe use of functional equipment, ensuring correct equipment is ordered, delivery and fitted correctly.
  • Complete medical observations as per agreed clinical pathways within scope of practice, for patients in their home.
  • Prescribe and install living aids: prescription of daily living aids for patients such as walkers and frames and train patients and families on how to use them safely.
  • Equipment advice: give advice on how a home or workplace environment could be adapted (e.g. wheelchair ramps, a stair lift or grab rails for safety) in order to afford a greater independence and safety.
  • Educate patients and give advice on the safe use of functional equipment, ensuring correct equipment is ordered, delivery and fitted correctly.
  • Where needed (e.g. at end of agreed care episode) refer patient on for relevant NHS and / or social services such as community OT.
  • Escalate issues/problems to Clinical Service Manager /Senior Clinical Services Manager where needed / clinically indicated.
This all sounds great - What key skills do I need to apply
  • Qualification at degree level (or equivalent) in Occupational Therapy.
  • Registered with the Health & Care Professions Council (HCPC).
  • Previous experience of working as part of a multi-disciplinary team.
  • Track record of delivering rapid response services.
  • Strong patient and customer focus.
  • I.T. Skills including email, excel.
  • Excellent communication skills, including being assertive when needed.
  • Planning & organisational skills - is able to multi task, and shows good time management skills.
