Job Description

Main Purpose of Post:To proactively manage the rehabilitation of service users in Custody and across the gate to support the overall objective to reduce reoffending and risk of harm on release. Ensure the coordination of HMP Stakeholder interventions ensuring they are arranged and delivered. That ITTG is delivered through the principles of the Interchange Operating Model, including the introduction to Interchange, assessment, management and planning of risk of harm, resettlement planning, arranging and delivery of interventions including non-accredited groupwork where appropriate. To liaise with community based partners to co-design sustainable rehabilitation networks. To demonstrate a personalised approach with a focus on enablers and strengths of the individual.This vacancy is based at HMP Hull and HMP HumberSpecific key responsibilities for High Intensity Transition Officers• Case management of specific CRC Through The Gate service users• Supporting rehabilitation across the gate into the community during the period of transition• Supporting the Responsible Officer in the event of service user relapse• Providing more intense 1-1 support during the custodial periodOperational Planning and Management• Identify and utilise relevant services for Protected Characteristic groups• Ensures that all activities will be undertaken in an enabling manner consistent with rehabilitation goals and desistance approaches to enhance consistency in delivery and coordination of interventions in the prison and across the gate• Responsible for motivating, monitoring and recording the service users progress through Custody interventions and support particularly during the critical last 12 weeks (or less)• Explain and introduce the Interchange Model to service users (including the six core modules) ready for community based CRC interventions• Deliver non-accredited groupwork activities• Refer to relevant interventions and services• Communicate with the community based Responsible Officers and community based partnerships to ensure a joined-up and co-designed approach for release• Maintain clear and timely record-keeping on the Case Management System• Conduct strength-based assessment, Resettlement Planning and SMART objectives whilst taking into account risk of harm and safeguarding concerns on release• Motivate service users to engage, comply and complete group work interventions• Optimise community based Networks towards sustained rehabilitation• Participate in multi-agency forums• Participate in multi-agency release planning• Liaise as required with the NPS, OMU, OMiC and Pathfinder (Extremism)• Demonstrate CRC values at all times• Comply with Prison regime requirements• Contribute to the development of the ITTG model• Promote and develop structured mentoring support that also helps build resistance to peer influences• Maintain a one team approach with current TTG providers• Meet the ITTG IQAM standards • When safe to do so, work with family members and significant others to develop community based networks• Increase positive outcomes for vulnerable and complex groups such as young men, Women.
