Thats when it hits home.We have a fabulous job opportunity for you to join our awesome team as a Support Coordinator. We support our brilliant customers who have different Mental health diagnosis.They live in a beautiful grade 2 listed building in the heart of Hexham, each customer has their own bedroom, and all share a communal kitchen. There is a two-bed cottage attached to our main building which consists of a large communal kitchen and a games area.We also have a growing STEP service where we support customers in their own home around the Hexham area.We help our customers with everyday living tasks, such as general housekeeping and meal prep. We also support our customers out in the community, whether that be attending appointments, shopping or days out. We also love a BBQ in the summer under the canopiesin our courtyard, karaoke nights and enjoy a communal Sunday lunch every week.Youll help change our customers lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. Youll create bespoke and personalised support plans with and for your customers.Each plan will lookto meet our customers individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements. Youll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.We empower our customers to develop the independent living skills they need, to successfully move on.
