Job Description

We currently have a permanent post for a Band 6 RMN 22.5 hours per week, in the Holderness Older People’s Community Mental Health Team, based at Rosedale Community Unit in Hedon. We are seeking a highly motivated and enthusiastic individual to work within an established and busy Community Mental Health Team. The applicant will be expected to have specialist nursing knowledge of working with older people with organic and functional mental health issues. As a senior clinical member of the team your duties will include acting as a care co-ordinator within the multi- disciplinary team and to provide education and advice to other professionals, clients and carers and manage a complex case load. You will assume delegated duties for the Band 7 Team Lead in their absence and deal with team issues, provide supervision of nursing staff and also cover for peers when absent. You will lead and participate in the development of staff through appraisal and clinical supervision. Participate and work alongside the Band 7 Team Leader to ensure team activity is monitored in accordance with the activity monitoring system. You will provide nursing interventions across teams in order to support the wider community services as required by patient need and demand. The applicant will be expected to have knowledge in Dementia and experience in this area would be an advantage, additional training and support will be given. You will have a good knowledge of current policy drivers and be able to implement research into practice with an understanding of the principles of Valuing People and Transforming Care. You will be key to implementing national and local policies within the service. You will have an in-depth knowledge of person centred thinking and be able to demonstrate this throughout your practice.
