Motor Neurone Disease Clinical Nurse SpecialistLocation: North Yorkshire Hospice Care Sites and communitySalary: £21,535.14 per annumHours: 22.5 hours per weekPosition Type: Fixed term temporary (1 year)Closing date: Thursday 16 December 2021An exciting opportunity for a Registered Nurse with specialism in end of life care and neurological conditions to deliver and co-ordinate an integrated and specialist service for people and their carers living with Motor Neurone Disease across the Harrogateand Rural District locality.Key responsibilities will include assessing and identifying clinical, psychosocial and spiritual needs of individual patients living with Motor Neurone Disease and, in partnership with patients/carers and other healthcare professionals, plan programmes ofcare to meet these needs.You will act as a collaborative key worker and advocate, referring patients to other professionals as required to ensure high quality appropriate care is provided. The successful applicant will be expected to contribute to the governance processes of theorganisation as well as assess training and development needs of colleagues and use appropriate strategies/opportunities to share knowledge and influence the practice of others working with people living with a progressive neurological condition.Holding a clinical caseload and working in collaboration with multi-disciplinary teams, you will provide direct specialist care, advice and support to patients and carers as appropriate from the point of diagnosis onwards and across all places of care.We are looking for a Registered Nurse with a current pin who has, or is willing to undertake, post-registration study/qualification in palliative care. The post-holder must have proven palliative care and neurological conditions experience working at Agendafor Change Band 6/equivalent or above and Experience of substantial medium/long term support to patients living with progressive neurological conditions and their carers.Benefits include: -
